Contest Rules and Information

One applicant will be selected to receive one fully covered bariatric surgery performed by Dr. Betsy Dovec at The Surgical Institute of Central Florida, including surgeon fees, facility fees, anesthesia, and standard post-operative follow-up care through BodyByBariatrics, subject to medical evaluation and suitability.

Eligibility & Entry

To apply, you must:

Be 18 years or older
Meet all clinical criteria for bariatric surgery
Submit a 250–500 word written entry explaining why you should be selected
Provide before photos and agreement to use your photos/story publicly
Complete the official application form
Consent to medical evaluation prior to final selection
Agree to participate in promotional content

Submission Deadline: Sunday, October 25, 2026 at 11:59 PM Eastern
Winner Announcement: Wednesday, October 28, 2026 at 7:00 pm EST
Winner must be present on the BodyByBariatrics Instagram Live

I understand that bariatric surgery is a medical procedure with risks and potential complications. If selected, I agree to undergo full medical evaluation, provide medical history records, and complete the informed consent process before surgery. This application does not guarantee medical approval for surgery.

Publicity Consent Checkbox

I grant BodyByBariatrics permission to use my name, written application, photographs, video content, and personal story in marketing, promotional, educational, and advocacy materials in any media format. I waive the right to inspect or approve final materials.

Photo Release Agreement

I understand that photos and videos submitted with my application may be publicly shared as part of the Scholarship selection and promotional process.

Travel Costs Checkbox

If selected, I understand that all travel, lodging, and personal expenses related to participating in the Scholarship and receiving surgical care are my responsibility.
