Standing on the edge of a life-altering decision, the line between a euphoric breakthrough and overwhelming fear feels razor-thin. It’s a space where excitement and doubt coexist, where the possibilities of transformation are just as vivid as the potential setbacks. I’ve seen this repeatedly through the eyes of my patients—the fear of the unknown and the powerful pull to take control of your life. In this moment, you’re torn between the life you know and the life you could have. And it’s terrifying. But it’s also exhilarating.
There’s something profound about the moment when you stand on the edge of a major life change. For so long, you’ve lived in a body that feels more like a barrier than a home, and now you’re staring at the precipice of bariatric surgery. You want more—more energy, more health, more life—but at the same time, you’re terrified. It’s as if you’re standing on the edge of a cliff, staring down into the unknown, wondering if you’ll fall or fly.
You know you can’t keep living like this. Something doesn’t feel quite right. Every step feels heavier, physically and emotionally. You can sense that there’s more to life, more happiness, more opportunities, but the idea of taking that leap into the unknown is terrifying. What if it’s a mistake? What if everything changes, and not for the better?
These thoughts are completely normal.
Will I fail?
This is the question that haunts so many of us. We’ve tried diets, exercise plans, and lifestyle changes, and maybe they haven’t worked—or they’ve worked for a while, only for the weight to come back. What makes this different? The fear of failure can be paralyzing, but it’s important to remember that bariatric surgery is more than just a tool for weight loss. It’s a decision to reclaim your life.
What will eating be like?
This fear of the unknown is huge. Will I still enjoy food? What if my relationship with food never changes, or worse, what if I miss it too much? Bariatric surgery isn’t just about restriction; it’s about transforming your mindset and your habits around food. This is such a common concern, we created Project Reset with live small group sessions that teach cognitive behavioral therapy strategies with Bariatric Therapist, Lora Grabow, to tackle Bariatric Grief and the Emotional Eating Cycle.
Patients are calling Project Reset life-changing, with many sharing how the support, accountability, and community have helped them break free from the cycle of self-doubt and guilt. Through guided sessions focusing on mindset, movement, and meals, you’ll gain the tools to reframe your journey and build lasting habits that truly align with your goals.

What will my friends and family think?
One of the hardest parts of this journey is worrying about how others will perceive you. Will they support you, or will they judge? Will they understand that this isn’t about vanity but about your health, your happiness, your future? At the end of the day, this is your journey, and while support is crucial, this decision is ultimately about your well-being—not anyone else’s opinion.
Will I lose my hair? Gain all the weight back? Follow the rules?
It’s normal to think about the physical changes and challenges that might come after surgery. Hair loss can happen, but it’s temporary, and, if it were to occur (usually if you’re not getting in enough protein), happens between months 3 or 4 post-op until 7 to 8. The great news – it grows back! Listen to our DoctorXDietitian Podcast about this potpourri of issues (and misconceptions about bariatric surgery!) with Dr. Betsy Dovec and Hannah Schuyler, RD.
Will you follow the guidelines, stay committed, and make the changes necessary to maintain your success? You’re not alone in this. Our BodyByBariatrics team, Facebook and monthly live and interactive support groups, and resources are there to guide you every step of the way. And yes, you can, and WILL, succeed.
What if I hate my choice?
This is one of the scariest questions because it speaks to the fear of regret. But ask yourself, what if you don’t do it? What if you stay in the same place, feeling stuck, unhappy, and unfulfilled? Sometimes the fear of change is overshadowed by the fear of staying exactly where you are.
But What If You Succeed?
What if this decision leads to the life you’ve always dreamed of? What if you love your new body, not just because of the physical changes but because of the confidence that comes with it? What if you finally have the energy to do the things you’ve been too tired or too self-conscious to try?
What if this leap of faith transforms not just your body, but your mindset, your habits, and your future? Imagine walking into a room and no longer worrying about what people think. Imagine making choices based on what you want, not out of fear of judgment or failure. Imagine living without the constant, nagging voice that says you’re not enough.
What If Your “Yes” Is Holding You Back?
Sometimes, the fear of change is so great that we say “yes” to staying in our comfort zones, even when those zones aren’t serving us. We say “yes” to unhealthy habits, to toxic relationships with food and self-image, to the life that feels like it’s slowly closing in on us. What if your “yes” is actually sabotaging your chance for true happiness and health?
What If You Regret Not Doing It?
Perhaps the biggest “what if” is this: what if you regret not taking the leap? What if, five years from now, you look back and wonder how your life could’ve been different? What if you miss the opportunity to transform your life because fear held you back?
Take the Leap
Yes, the unknown is scary. Yes, there will be challenges. But there is also the possibility for immense growth, healing, and change. Bariatric surgery is not just about weight loss—it’s about stepping into the life you’ve always wanted but didn’t think was possible. It’s about saying goodbye to the fears, doubts, and insecurities that have kept you from living fully.
You’re standing on the edge, looking down. It’s time to take the leap, not into the unknown, but into the possibility of a life that is finally yours. Don’t let fear decide your future.
The decision is yours. The journey is yours. And the life you want? It’s waiting on the other side.
